BMI (Body Mass Index) is one of the most used and most criticised health metrics in existence. Doctors use it. Insurance companies use it. Critics say it's meaningless. Body fat percentage is considered more accurate — but almost nobody actually knows theirs.

So which one should you be tracking, and why does it actually matter?

What BMI Actually Is

BMI was invented in the 1830s by Belgian mathematician Adolphe Quetelet — not a doctor, a statistician. He created it to describe population weight distributions, not to assess individual health.

The formula is simple: BMI = weight (kg) ÷ height (m)²

It tells you one thing: how heavy you are relative to your height. That's it. It knows nothing about whether that weight is muscle or fat, where fat is distributed in your body, your age, your sex, your ethnicity, or any of the other variables that actually drive health outcomes.

Where BMI Goes Wrong

BMI fails in three specific and well-documented ways:

1. It can't distinguish muscle from fat. Muscle is denser than fat. A powerlifter or rugby player with 12% body fat will often register as "overweight" or even "obese" on BMI. Conversely, a sedentary person who has lost muscle mass with age may have a "normal" BMI while carrying dangerously high body fat.

2. It ignores fat distribution. Fat stored around the abdomen (visceral fat) carries significantly higher health risks than fat stored around the hips and thighs (subcutaneous fat). Two people with identical BMIs can have very different risk profiles depending on where their fat sits.

3. It has different risk thresholds by ethnicity. Research shows Asian populations face elevated metabolic risk at lower BMI values. Several health organisations now use adjusted thresholds for Asian patients: overweight at 23+ and obese at 27.5+, rather than the standard 25 and 30.

⚠️ "Normal weight obesity": It's possible to have a BMI in the healthy range (18.5–24.9) while having a body fat percentage in the obese range. This is more common than most people think, particularly in older adults who have lost muscle mass.

What Body Fat Percentage Tells You

Body fat percentage is a direct measure of what matters: how much of your body weight is fat versus lean mass (muscle, bone, organs, water).

The Problem With Body Fat Percentage

Body fat is more meaningful — but it's harder to measure accurately:

The most common consumer-grade tools (bathroom scales with impedance, fitness trackers) give you a number, but that number can swing by 3–5% based on when you weigh yourself, how hydrated you are, and what you've eaten. Don't over-interpret single readings.

Which Should You Track?

The honest answer: both, with context.

BMI is useful as a quick, free, population-level screening tool. If your BMI is 19–24, you're unlikely to have a body composition problem. If it's 30+, there's high probability of elevated health risk. It's imperfect, but it's a useful first filter.

Body fat percentage gives you meaningful information BMI can't — especially if you exercise, are older, or have an athletic build. If you track your body composition over time, body fat percentage change is a far better indicator of whether your diet and exercise approach is working.

The best approach for most people:

  1. Use BMI as a rough starting point

The Number That Matters Most

Ultimately, the best health metrics are the ones your doctor actually uses to assess risk: blood pressure, fasting glucose, HbA1c, cholesterol profile, triglycerides. These are downstream of body composition — but they're what actually drives heart disease, diabetes, and early mortality risk.

BMI and body fat percentage are proxies. Useful, accessible, imperfect. Don't obsess over a single number on either scale. Track trends over months. Focus on behaviour (exercise, diet, sleep) rather than the metric.
